Are you seeing "Gamer, Advanced, Hardcore" and not "High, Very High, Extreme" in Settings? This is why.
Retail untouched exe (with latest patch) shows "High, Very High, Extreme". kracked/Fairlight exe shows "Gamer, Advanced, Hardcore" (I got the kracked exe just to test this). So all those saying they are not seeing "Extreme or the other settings", go buy the game or use the retail exe (if you don't want your cd to get scratched). Here is a video showing the difference I put together. The first half shows the kracked exe settings, the 2nd half shows the retail.
