i'm super newbie here

my english is very poor, i bet there'll be so much errors in my post
i feel not confident using english, because of lack of skill
i hope you don't mind it, and please help me to correct it

i've completed toefl pbt test twice
i scored around 580 of both tests
I got the highest score among my friends
but that was all that I could, doing passive English test with multiple choice, and i think it's useless

i'm really having difficulties in active english, my toefl score doesn't reflect my english ability at all

I find it difficult to form sentences because there's missing vocab that I do not know, or I actually know but not unthinkable, I also find it difficult because i'm not mastered grammar, and it's hard for me to mastered it, even i've studied the theories, like sentences.. the form.. past present future etc.. but i still confused to use it practically,, for example i want to make a sentence but i don't exactly know what theories that i should use,

I really want to be able to speak and write fluently and confidently in English, i wanna ask you what is the right method for people like me, and i'm sure that the answer is to practice and make it habbit but i don't know where to start should i take an english courses, or i can practice it myself

ah i really embarassed because it took so much time for me to write this post
